Frequently Asked Questions

Primary care physicians (PCP's) direct and coordinate all aspects of your healthcare. They listen to your questions, assess symptoms, recommend treatment approaches, prescribe medications, and refer you to the proper specialist when need be. 


It is important to build a long-term relationship with a PCP, as this will translate into health benefits over your lifetime, such as a decrease in hospital and ER visits and better management of chronic diseases. 


A Medicare Annual Wellness Visit is an appointment with your primary care provider during which he or she will update your prevention plan. The purpose of the appointment, after all, is to help you prevent sickness based on your health status and any risk factors you may have.

This appointment with your doctor is not the kind of full-body physical that you may have had in the past. Instead, it’s more of a review with your doctor so that he can go over a written checklist and design a prevention plan for you.

Here are some of the covered items that your provider is likely to review with you during your Medicare Annual Wellness Visit:

  • Routine measurements
  • Height, weight, and blood pressure check
  • Health risk assessment consisting of questions about your healthcare status, needs, and risks
  • Review of your medical history and family medical history
  • Check your list of current medications
  • Assessment of your functional ability and mobility 


Universal Medicine is in-network with most major insurance carriers. Please be sure to contact your insurance company directly to confirm whether or not we participate with your plan. 


Please be sure to bring your insurance card, identification, pertinent medical records, a list of your medications, and any imaging and/or laboratory results with you to your appointment. 


If you have chosen to complete the patient intake forms at the office, please be sure to arrive at least 15 minutes before your scheduled appointment in order to prepare for your visit. We highly recommend submitting your intake forms in advance via our website. 


If you are unable to keep your appointment, please be sure to contact our office directly at (201) 308-8995 to notify us as soon as possible. If you miss your appointment and do not call to cancel, additional fees may apply.
